I filled out the form with my name and address, and in the message section requested the personalized signed photo.
I sent the request months ago and received it today in a cardboard envelope. Very sturdy! The show paid the postage and supplied the photo.
I'll try to attach the photo here, but frequently I get error messages saying that this website's forum attachment quota has been reached.
